On this (21st) day of (March), A.D. 18(66), before me (Edwin Fuller Judge) of the (Probate Court) in and for the County and State above named, personally appeared (David H Lee), a resident of (Bowling Green), in the County of (Wood), and State of (Ohio), aged (57) years, who being first duly sworn according to law, doth on oath make the following declaration as guardian of the minor child(ren) of (John Crom), deceased, in order to obtain the benefits of the provision made by the act of Congress approved July 14, 1862, granting pensions to minor children under sixteen years of age of deceased officers and soldiers: That he is the guardian of (Eddie L. Crom, who was born December 27, 1860, and of George Crom who was born August 19, 1862) whose father was a (Private) in Company (("K")) commanded by Captain (S.S. Canfield), in the (21st) Regiment of (Ohio) Volunteers, in the war of 1861, and that the said (John Crom) died at (Chattanooga Tenn) on or about the (Twenty first) day of (March, 18(64), by reason of (Disease supposed to be Measles.) That the mother of the aforesaid child(ren) (re-married on the 24th day of August 1865 to Samuel Akley as shown by the accompanying affidavit) that the date of birth of his said ward(s were) as follows: (Eddie L. Crom who was born December 27, 1860, and George Crom, who was born August 19, 1862) He further declares that the parents of his said ward(s) were married at Bowling Green Ohio), on the (20th) day of (January), 18(60) by (Samuel L. Roberts, Minister of the Gospel); and he also declares that he as not in any manner been engaged in, or aided or abetted the Rebellion in the United States. Woodbury of Bowling Green, Wood County, Ohio) his Attorney to prosecute this claim for a Pension and procure a Certificate, and revokes and countermands all former authority that may have been given for the above specified purpose. That their knowledge of the marriage of the said parents of the date of birth of said child(ren) and of the (re-marriage) of the mother is derived from (intimate acquaintance with John Crom deceased and his children, and also with Samuel Akley the present husband of the Mother) and they further testify that they are disinterested in this claim for a Pension.
