Notes: The deceased was engaged withdrawing the settings of timber in an old cross-road in the Shallow seam. While cutting out a prop to release a bar of timber the prop suddenly collapsed, and a heavy fall of roof took place from between a slip and a break which killed him instantly. A sylvester pulling jack was provided, close at hand, for the purpose of withdrawing the timber. The use of an axe to cut out timber was prohibited.7
