I have some information on my g-gf's grave at FSH. I'm not sure if you're building a database for Cemetery 4 and 5. I got the following information from the Florida State Hospital, Central Health Information Services, PO Box 1000, Chattahoochee, FL 32324-1000, Senior Clerk, Deborah Germany on May 1, 2000.

Section "A" West-South, Row 3, FSH Grave No 225
His personal information is as follows:
William Warren Giddens (Giddiens <-- correct spelling)
June 1853-August 28, 1934 "resident of the institution" from Feb 24, 1932 until his death.
