We lost our poem ages a couple of generations back. Most people I think just have the grandfather or oldest male relative on the father's side name the children.
My husband generation has the generational name, but his dad wasn't too traditional and told the sons to name the kids whatever they wanted. Of course he passed away b/4 he had any grandsons. I don't know if it would have made a difference to his dad if he had grandsons first. None of the granddaughters followed a generational name.
