1.    I cannot read the name.  The handwriting is too poor.

2.    My best effort to read the name leads me to two (or more) possibilities.  It could be (a) or (b).

3.    The writing is clear but the spelling appears to be different from the usual spelling of the name.

4.    I checked the name with the handwritten name on the designated page and found the typed index name to be in error.
