Transcription of some of the text:

"I certify on honor that Phineas H Blood was a sound able-bodied man at the time of his enlistment
and so continued until on or about the 1st of June 1862 when he became unfit to perform the duties
of a soldier because of diarrhea."

"Chronic diarrhea and general debility. Said Blood has been unfit for duty for a long time and is not
likely to be good for any thing.  -Enoch Blanchard, Surgeon"
