Transcription of some of the text:

"I certify that I have carefully examined the said Private Phineas H. Blood of Captain John Chase’s Battery
and find him incapable of performing the duties of a soldier because of Rheumatic Sciatica affecting back
and left thigh. Has lasted nearly a year and has resisted treatment. Seems to be caused by some irritation
of the sciatic nerve within the pelvis. Disease was not contracted in line of duty. He is physically unsuitable
to enter the Disabled Corps. Degree of disability two thirds. He can not walk or march with his company."
