Know ye, That Adam Burckhalter, a private of Captain AG Trevett, Company A, 98 Regiment of NGI - NY militia, who was enrolled on the tenth day of August, one thousand eight hundred and sixty-four to serve (illegible) days or during the was is hereby DISCHARGED from service from the United States, this twenty-second day of December, 1864 at Buffalo by reason of having served his time. (No objection to his being re-enlisted is known to exist)
Said, Adam Burckhalter was born in Germany, in the state of (Blank), is 28 years of age, 5 feet, 6 3/4 inches high, fair complexion, blue eyes, Light hair, and by occupation, when enrolled, a Farmer.
Given at Buffalo, NY, this twenty-second day of December, 1864.
