CHAPTER XVIII
Purefoy's appointment and career-The Pathological
Laboratory-Develop-ment of the Extern Maternity and Out-patients'
Departments
RICHARD DANCER PUREFOY, who succeeded Smyly, was born on 4th August
1847 at Cloughjordan, Co. Tipperary. He was a son of a Dr. Thomas
Purefoy of Lucan, Co. Dublin, who had married a daughter of Thomas
Dancer, of Hilton, Co. Tipperary. The Purefoy family came from Fenny
Drayton, Norfolkshire, in 1620, and settled at Woodfield, Co. Galway.
Purefoy was educated at Bective College and Raphoe Royal School. From
the latter he entered Trinity College, Dublin, graduating with Honours
in the autumn of 1871. He won a Junior Moderatorship in Natural Science
along with his class-fellow, friend and colleague Smyly.
In 1871 Purefoy obtained the Licence in Medicine and Surgery of the
University of Dublin, graduating in Arts, Medicine and Surgery early in
the following year. In 1875 he took the Licence, or "Letters
Testimonial. " of the Royal College of Surgeons in Ireland, proceeding
to the Fellowship of that college in 1879. It was not till 1892 that he
became a Doctor of Medicine of his Alma. Mater, which, twenty years
later, conferred upon him the degree of LL.D. Honoris Causa, on the
auspicious occasion of the bi-centenary celebration of the School of
Physic of Trinity College. At that time he was President of the Royal
College of Surgeons in Ireland.
Purefoy had been Assistant Master in the Coombe Hospital before
becoming Assistant Master in the Rotunda. In 1879 he was appointed
Lecturer in Materia Medica in the Ledwich School of Medicine, in Peter
Street, Dublin. He also acted as Obstetrical Surgeon to the Adelaide
Hospital for twenty-one years. Purefoy's Presidency of the Royal
College of Surgeons, Ireland, was from 1912-1914, and in 1915 he was
chosen as President of the Royal Academy of Medicine, Ireland. He died
at 62 Merrion Square, Dublin, on 27th June 1919
The Rotunda Hospital
Purefoy was Assistant to George Johnston from 10th July 1874 to 13th
July 1877, and was Master from 1896-1903. He encouraged the expansion
of the Extern Maternity Department, and appointed an additional
Clinical Clerk, whose chief duty was to supervise the maternity work on
the district, while assisting in the new Gynaecological Hospital
(Thomas Plunkett Caimes wing) and superintending the work in the
Pathological Laboratory. Because of this new appointment the status and
salary of Clinical Clerks were raised in 1897, and then, as now, they
rank next to the Assistant Masters.
The opening of the first Pathological Laboratory in the history of the
hospital was a definite advance. Purefoy established and equipped it as
a gift to the hospital. By organising a successful "Lucina" bazaar he
had collected money which, for the time being, largely freed the
Rotunda from debt. Purefoy was held in high esteem by the nursing
staff, and the bronze bust by Oliver Shepherd was presented by them,
and now stands in the entrance hall of the hospital.
By developing the Extern Maternity service Purefoy relieved the
congestion within the hospital, and provided further opportunities for
the teaching of students. The Extern Maternity provides the student
with most valuable experience for he is there confronted by the real
problems, and his self-confidence and knowledge grow apace.
Eleven thousand and ninety-eight women were delivered in the Rotunda
during Purefoy's seven years, with 42 deaths - a mortality rate of 0.37
per cent.
Although the Out-patients' Department (Dispensary) had been working
satisfactorily for some time, it was first mentioned in the Report of
1898-1899; the total number of attendances for the year being
10,168.-Gynaecological outpatients were then being treated in the
general hospitals, thus Purefoy's greater attention to this essential
part of the Rotunda was justified.

PUREFOY.
R.D. Purefoy was born in Tipperary on the 8th August, 1847. He is the
fourth son of the late T. Purefoy, of Lucan, by his wife Alla Maria,
daughter of Thomas Dancer, of Hilton, Co. Tipperary. His education was
conducted at Bective College, Rutland-square, and Raphoe Royal school.
He entered Trinity College, and passed through a distinguished
undergraduate course, winning a Moderatorship in Natural Sciences and
Musical Exhibition. His professional studies were pursued in the School
of Physic and Sir Patrick Dun's and Meath Hospitals. He served for two
years as House Surgeon in St. Mark's Hospital, and acted as Assistant
to the Master for three years in the Rotunda Hospital, and for a
similar period in the Coombe Hospital.
Dr. Purefoy graduated in Medicine in 1872. In 1875 he took out the
"Licence" in Surgery of the University, and also the Licence in
Medicine. At present Dr. Purefoy is Obstetric Surgeon to the Adelaide
Hospital, and Lecturer on Materia Medica in the Ledwich School of
Medicine. He has contributed several papers to the medical journals.
Dr. Purefoy is not married.

Dr. R. D. Purefoy died at his residence in Dublin on June 27th in his
seventy-second year, after a brief illness. His friends had noticed for
some time that his health was failing, but it was not until a month or
two ago that he admitted any loss of vigour. Early in June he went to
Newcastle, co. Down for a holiday, but he felt so ill that he came home
in a few days. He was carried to his bed and gradually sank to rest.
Richard Purefoy came of a medical family, his father practising as a
doctor at Lucan, a few miles from Dublin. His mother was a native of
Tipperary, and it was in that county that he was born. Educated at
Raphoe School, Co Donegal, and Trinity College, Dublin, Purefoy took
his medical degrees in 1872. In 1879 he became a Fellow of the Royal
College of Surgeons in Ireland. From the first he devoted himself to
the obstetric art. and was successively assistant master at the Coombe
and at the Rotunda hospitals. In the latter he worked under Lombe
Atthill and George Johnston. He was for many years gynaecologist to
the Adelaide Hospital, and in 1896 he returned to the Rotunda as
Master. "-When his term of membership was ended he was elected
consulting gynaecologist to the hospital. He signalised the termination
of his office by presenting to the hospital a fully equipped clinical
laboratory. For many years he was very busy in practice, and for a
genera-tion he was one of the leading obstetricians and gynaecologists
in Ireland. In 1912, after many years service as a member he was
unanimously elected President of the Royal College of Surgeons, being
the first of his specialty to occupy the chair since 1880, when
McClintock was President. - It was during his tenure of office as
President that Purefoy received the honorary degree of LL.D. from the
University of Dublin at the Bicentenary celebrations in 1912. In 1915
he was elected President of the Royal Academy of Medicine.
By Purefoy none of the many posts of honour he held were regarded as
sinecures. He fulfilled their duties in most exact manner. The Rotunda
Hospital was, however, his chief interest in life. No governor in the
history of that great charity ever held it in closer affection or
devoted mere time and energy to its welfare. Outside his professional
work Dr. Purefoy was interested in music and art. Possessed of a
baritone voice of good quality, in his younger days he sang in the
choir of Trinity College Chapel, and all his life he was a member of
many of the musical clubs in Dublin. At the time of his death he was
President of a very ancient musical society-the Hibernian Catch Club,
and he also collected pictures, glass and furniture. In private a
generous but keen student of life, few men of his age kept up so much
interest in the advance of knowledge. Masterful in his individuality,
he did not bear opposition easily, and while no one could be a better
friend he was frank in his dislikes. His disappearance leaves a gap in
the professional and social life of Dublin where he was for so long a
characteristic and prominent figure.

One-time Master of the Rotunda Hospital, Dublin.
DR. RICHARD DANCER PUREFOY died at his residence in Dublin on June
27th. He was a son of the late Dr. Thomas Purefoy of Lucan, and was
born on August 4th, 1847. He was educated at Bective College and Raphoe
Royal School, and entered the Dublin University, where he obtained the
licences in medicine and surgery in 1871, and graduated BA. and M.B.
with honours and a moderator ship natural science in 1872. In 1875 he
took the licence of the Royal College of Surgeons, and the degree of
M.D. in 1892. He was a member and past President of the Royal Irish
Academy of Medicine. At the bicentenary of the Medical School the
University of Dublin conferred on him the honorary degree of LLD. He
was a Fellow and member of the Council of the Royal College of Surgeons
and president 1912 to 1914, and a member of the Royal Dublin Society.
Dr. Purefoy was for five years house-surgeon at St. Mark's
Ophthalmic Hospital and for three years assistant master of the Coombe
Lying-in Hospital. He was then appointed assistant master at the
Rotunda Hospital. In 1879 he was appointed lecturer in materia medica
to the Peter Street School of Medicine. He was obstetric surgeon to the
Adelaide Hospital for twenty-one years, and acted as examiner in
midwifery and gynaecology in the University of Dublin. From 1896 to
1903 he was master of the Rotunda Hospital. While there he threw
himself heart and soul into the clearing of the hospital debt by
organising a big bazaar. On the termination of his master-ship he was
unanimously elected a governor of the hospital. And to-day a bronze
bust by Oliver Shepherd, presented to him by the nursing staff, stands
in the entrance hall. Dr. Purefoy was an old member of the Grand
Master's Lodge and a member of the Friendly Brothers' Club. Last
December he was elected president of the Catch Club.
He was a man of deep sympathy, never sparing himself, giving the
greatest care and kindly attention to the poorest of his patients. His
death will be felt as a great loss not only among his colleagues but
also among all ranks in Dublin.

DEATH OF EMINENT DUBLIN DOCTOR
We regret to announce the death which took place .yesterday, at his
residence, 62, Merrion Square, of R. D. Purefoy, LL.D., M.D., FR.C.S.I.
The late, Surgeon Purefoy was the fourth surviving son of Mr. Thomas
Purefoy, M. D. of Lucan. Born on the 4th of August 1847, he received
his early education at the Royal Raphoe School. Later at Trinity
College his splendid voice won for him a musical exhibition prize. His
professional qualifications secured for him the appointment of
Assistant Master of the Coombe Hospital. There he attained remarkable
skill, and his unique experience in midwifery led to his appointment
later as Master of the Rotunda Hospital. Genial in manner, he was held
in the greatest esteem amongst the wider circle which included his
clients, as well as the public generally. A great lover of horses, the
fast animals he drove were always a notable feature of his equipage.
Dr Purefoy took a great interest in archaeology, and he was one of the
first members of the Georgian Society. He made a special study of
botany, and, indeed was a man of very wide attainments. At the
tercentenary celebrations of Trinity College he received the degree of
Hon. D. Litt. He was a Fellow, and a past President of the Royal
College of Surgeons. Surgeon Purefoy was one of the most distinguished
members of the medical and surgical profession in Ireland. He was
consulting gynaecologist to the Rotunda Hospital, and while enjoying a
wide practice, managed to obtain the time to become the possessor of a
large and valuable collection of rare china and "objets d'art."

LL.D., F.R.C.S.I., who died at his residence, 62 Merrion square,
Dublin, on Friday of heart failure. Dr. Purefoy, who was the fourth
surviving son of Dr. Thomas Purefoy of Lucan,. was born on the 4th of
August, 1847, at Cloughjordan, and was educated at Raphoe Royal School.
He entered Trinity College where at the bi-centenary celebrations of
the Medical School5 he received the degree of LL.D. He was most
distinguished in his medical pursuits, and was appointed Assistant
Master of the Coombe Hospital. His skill and ability later secured for
him the position of Master of the Rotunda Hospital, to which he: was
consulting gynaecologist. He was greatly interested in archaeology, and
was one of the, original mem-bers of the Georgian Society. A Fellow and
a past President of the Royal College of Surgeons in Ireland, Dr.
Purefoy occupied high rank in the profession of medicine and surgery'
in this country, and be enjoyed a very wide practice. He was also
greatly interested in botany, a science of which he made a special
study.

With much regret we record the death of Dr Richard Dancer Purefoy, of
Dublin, who for a generation past was one of the leading obstetricians
in Ireland. He had been in failing health for some weeks, but his
condition gave no cause for alarm until within a few days of his death.
He died on the evening of the 27th June. Dr. Purefoy was the son of Dr.
Purefoy, of Lucan Co. Dublin, and was educated at Trinity College,
Dublin. He was successively Assistant Master in the Coombe and the
Rotunda Hospitals, and sub-sequently for many years gynaecologist to
the Adelaide Hospital. In 1896 he was elected Master of the Rotunda in
succession to Sir William Smyly. At the termination of his office in
1903 he built, equipped, and presented to the hospital a clinical
laboratory. He was subsequently elec-ted Consulting Gynaecologist to
the Rotunda Hospital, and during the absence on active service of the
present master, Dr. Henry Jellett, from 1914 to 1917, he was one of the
Committee of three ex--Masters which carried on the duties of the
Master. Dr. Purefoy also took, much interest in the affairs of the
Royal College of' Surgeons. Admitted to the Fellowship in 1879, he was,
after many years' service as a Member of Council, elected President of
the College in 1912. In the same year, at the Bicentenary celebrations
of the School of Physic, the Honorary Degree of LL.D. was conferred on
him by his own university. In 1915 he was elected President of the
Royal Academy of Medicine In Ireland. He was a keen student of his
specialty, and he made frequent communications on subjects of obstetric
and gynaecological interest to the medical societies. Many of these
have appeared in our columns. Apart from his professional work, Dr.
Purefoy was possessed of much taste in music and the arts. He had a
fine baritone voice, and was a familiar figure at musical dinners and
other gatherings of musical people. At the time of his, death he was
President of what is probably the oldest musical society in the
Kingdom, the Hiber-nian Catch Club. He was a judge of painting, and he
had "a large collection of pictures, furni-ture, and china. He was a
man of strong character, with strong. affections, by no means free of
prejudices. In private life he practised the greatest generosity to his
friends, and in public affairs he was most punctilious in the discharge
of his duties. As President of' the College he performed every duty
with the utmost exactitude, while for many years past his care for the
Rotunda Hospital was one of the chief interests of his life. At the
time of his death he was in his seventy-second year. He was unmarried.

From his obituary"Dr. Purefoy has been for many years Medical Officer
to the Lucan and Leixlip Dispensaries. Previous to his obtaining that
appointment he was Physician to the Clough¡jordan Dispensary and Fever
Hospital, so that during almost the whole of his professional career,
extending as it did over a period of thirty-six years, he has been
connected with Poor-law appointments. He succumbed at last, however, to
phthisis, contracted during exposure to the hard¡ships of dispensary
practice. Had he retired from these duties some months ago, and gone to
live in a warmer climate, as his medical advisers recommended, he might
still live to
ornament the ranks of our profession; but the fact that as yet there is
no retiring pension given to Poor-law Medical Officers who have devoted
their energies during the best years of their lives to the service, may
perhaps have decided him to remain at his post and die in harness. We
hope that this state of things will
not long continue. Meanwhile it is our painful duty to record that a
kind friend, an accomplished physician, and a Christian gentleman has
departed from amongst us".
