Observations on the Quality of His Majesty's Ship [_____________]

Her best Sailing Draft of Water, when victualled and stored for Channel Service, being given this [__] Day of [______] 18 [__] Afore [__] [__] or as much lighter (at the same Difference] as she is able to bear sail.

Query the 1st How she behaves close haul'd, and how many knots she runs   In a Top-gallant Gale
  In a Topsail Gale
  How she steers and how she wears and stays

And query, whether she will stay under her courses
2d In each circumstance above mentioned (in sailing with other ships, in what proportion she gathers to windward, and in what proportion she fore reaches, and in general her proportion of lee-way
3d How she proves in sailing through all the Variations of the Wind from its being a point or two abaft the Beam, to its veering forward upon the Bow-line in every Strength of Gale, especially in a stiff Gale, and a head Sea; and how many Knots she runs in each circumstance; and how she carries her Helm
4th The most Knots she runs before the Wind ; and how she rolls in the Trough of the Sea
5th How she behaves in lying Too or a Try, under a Mainsail, and also under a Mizen ballanced.
6th What for a Roader she is, and how she careens ?

7th If upon Trial the best sailing Draft of Water given as above should not prove to be so, what is the best sailing Draft of Water ? Afore [__] [__]

13th How she stows her Provisions and Water, and what Quantity of the latter she carries, with four and six Months Provisions ; also the Quantity of Shingle or Iron Ballast which may be put out, when she is victualled for six Months  
14th The Weight of the Provisions taken on Board, in consequence of being stored for the above time
