Sick Ticket.

To the Commissioners appointed to take Care of Sick and Wounded Seamen, or their Agent at [______]

Whereas we have sent on Shore the Person mentioned in the within Ticket, being afflicted with [______]
we desire that Care may be taken to promote his Cure, and to provide him Quarters,
according to the Methods of the Navy.

If the Person in whose Favour this Ticket is made out shall be sent sick to any Hospital,
or Sick Quarters at Home or Abroad, and shall afterwards return to his ships he shall
leave this Ticket with the Agent of such Hospital or sick Quarters.

If he shall be sent from such Hospital or sick Quarters to serve in any other ship ;
he shall carry this Ticket with him, which shall not, be payable until the ship on Board of which he goes
shall come in course of Payment, or until he shall be discharged therefrom.

If he shall be removed from one ship to another Abroad, he shall deliver it,
when he arrives in England, to the Commissioners of the Navy in London,
or to the Commissioner of the Navy residing at Portsmouth, Plymouth, or Chatham,
who will cause Payment to be made as soon as may be, at whichever of these Places it shall be delivered.

If he shall be in any Way discharged unserviceable, he may receive Payment in the same manner
at any of those Places ; or if being so discharged unserviceable, he shall be or arrive at any other Port or Place in Great Britain or Ireland,
and shall be desirous to receive Payment at such place, of this or any other ticket which he may have, he shall apply to the nearest
Collector of the Customs or Excise, or Receiver of the Land Tax, or Clerk of the Check, and shall deliver to him such Tickets, together
with a Certificate of his having received no Wages for his Services during his passage, if he shall have come Home in packet or Merchantman ;
and the said Collector of the Customs, or whichever of the above Revenue Officers he shall apply to, shall transmit the same to the
Commissioners of the Navy in London, who will forward a Bill for the Amount of the Wages, to be immediately paid to the Person
entitled to them ; and the said Revenue Officer shall also transmit such Person's Address, who must be identified to him by the Master of
the said Packet or Merchantman, of by some other person of good repute.

N. 8. The personating or falsely assuming the Name and Character of any Petty Officer, Seaman, or Marine,
or procuring any other to do the same, in order to receive soy Pay or Wages due, or supposed to be due, is made Felony without
Benefit of Clergy, by Act 32 Geo. III. And if the Person in whose Favour this Ticket is made out, shall die before be receives
Payment it is to be returned, together with any other Tickets he may have in his Possession, to the Commissioners of the Navy, London.

Navy Office

To be paid out of Money received by the Right Honourable the Treasurer of His Majesty�s Navy for the payment of Seamen�s Wages.

To the Treasurer of His Majesty�s Navy.

If the Party shall be discharged unserviceable, be turned over to any other ship in the service of His Majesty,
of be sent sick, and do not return to his ship, the Captain or Commanding Officer is to send a Duplicate to the Commissioners
of the Navy, in London, by the first safe Opportunity hereafter.

Or any Captain or Commanding Officer, who shall have such Duplicate in his Possession upon his Arrival in England
is immediately to forward it to the Commissioners of the Navy, in London.

To be paid out of Money received by the Might Honourable the Treasurer of His Majesty's Navy, for the Payment of Seamen's Wages.
