These are to certify that [_________], Surgeon of [_________], voluntarily maketh oath, that the time of entry and discharge, death, and run; of all the men named In the annexed account, between the [_________], and the [_________], are right and true, and that none of the said people have been kept a day longer under cure than was necessary for the recovery of their health and cure of their wounds nor discharged a day sooner than he judged them fit to return to their duty, to the, best of his knowledge and belief, except the persons named and numbered in the margin; the reasons whereof are noted against their respective names.
