Age 22, single. Resides at North Judson, Ind. In case of injury, notify father, John H. Smith at North Judson, or brakeman E.C. Smith. Employed as car inspector since Aug. 5, 1895, transferred to Transportation Department and employed as brakeman Aug. 30th, 1897.

His attention called to Rules 202 to 208, also to Rule 228. Granted leave of absence for 30 days from April 13th, 1900, above leave of absence extended to June 12th, 1900, May 11th, 1900. Failed to return at the expiration of above leave and under date of June 13th, 1900, sent Trainmaster his resignation.
