Age 24 years, single. Residence Alliance, Ohio. Father's name John West, same address. Formerly with P&W Ry as brakeman. Has good letter from D.W. O'Neal, trainmaster of the P&W. Enters service of the C&E as brakeman, Sept. 11th, 1891.

Suspended 15 days for connection with collision of 1st 85 and 1st 82 at Palmer, March 18, 1892. Suspended 10 days from Oct, 21st for not being on top of his train approaching Wilders, resulting in 2nd 94 of Oct. 12, 1893 colliding with a Monon freight on the crossing.

Dismissed May 23rd for violation of Rule 202 at Chicago May 20th and Huntington, May 22nd, 1894.
