Age 22, single, resides at Huntington, Ind. In case of injury notify father M.F. Smith at 58 Frederick St., Huntington. No previous experience, except put in all the time since Oct. 5th learning the business. Enters the service at freight brakeman, Oct. 20th, 1899. His attention called particularly to Rules 202 to 208, both inclusive, also Rule 228.

Dismissed April 28, 1900 to reduce the force. Reemployed as brakeman on the Chicago Division, August 28, 1900. Discharged July 10th, 1901 to reduce the force. Reemployed July 22nd, 1901 as brakeman, Chicago Division. Resigned Oct. 27, 1901, File 5809.
