WILLIAM H. HORTON
Age 28. Single, resides with his parents at North Judson. In case of injury notify his father at North Judson. Formerly employed as clerk at North Judson, Hammond and Griffith for 2 years. Transfered to the transportation department and placed as extra freight brakeman, Oct. 13, 1899. His attention called particularly to Rules 202 to 208, both inclusive, also Rule 228 on Oct. 15th, 1899.

While making a coupling at Hammond, October 19th, 1899, had his arm caught between bumpers and seriously injured, and on his recovery decided that he had braking enough. Resumed work as clerk at North Judson.
