You may use this form to send individual or family information for inclusion in any of our family files. This form is sent by e-mail. If you would like to send digital pictures in jpg format, attach them to a regular e-mail message with identifying information about the subject(s) of the photo(s). Any information that you share with us is appreciated. None of the boxes are required to be filled in; if the source for any particular bit of information is your personal knowledge, just leave the source box blank. We make it a policy not to show detailed information on living individuals in the ONLINE databases, only the names of those individuals. If you do not want the NAMES online, please let us know when you send the information. If you find any errors in the information included in our databases, please notify us and provide the correct information so we may revise our databases accordingly.
