• Given Name:  variations of spelling may be included
• Date of Birth: As much of the date as known. MM/DD/YYYY
• Place of Birth: As much as known.
• Related Website: The Internet address of a web site related to this name. (If none, leave blank.)
• Supplemental Information: Supplemental information related to the brick wall name. Anything the submitter feels may be of value to viewers of the information.
• Submitter:  The submitter's name. On the web site, this becomes the link to the submitter's email address.
