Family Group Sheet Form

This form is used to submit individual or family information for inclusion in our database and this web site. Once you fill it out, it is sent to me by e-mail.

(If you would like to send digital pictures, attach them to a regular e-mail to us with any info about the subject of the photo).

Any Information that you can send is appreciated, none of the boxes are required to be filled in; if the source for any particular bit of information comes from your personal knowledge rather than a documented source such as a birth certificate, just leave the source box blank. You can submit the forms several times if multiple marriages are involved, etc.

All information submitted will be presented on the Internet. Please keep this in mind when submitting the data. Some people may not want the information on them published in such a manner. and we appreciate and respect their feelings. If you have any questions, please discuss them with me or with the involved persons.
