Appears on company Muster-in Roll of the organization named above. Roll dated Benton Barracks, Mo. Sept 28, 1862. Muster in date Sept. 28, 1862. Joined for duty and enrolled: Aug. 29, 1862, Eagleville for period of 3 years.

Appears on Company Muster Roll, Oct. 31, 1862 thru April 1865. Reported sick in May, June, July and August of 1863. On daily duty as cook during Nov. and Dec. 1863.

Appears on Co. Muster-out Roll, dated Little Rock, Ark. Jan 28, 1865. Muster-out date- Jan 28, 1865. Last paid to- Feby 28, 1865. Clothing account: Last settled Oct 31, 1864. Am't for cloth'g in kind or money adv'd $22.44.

Declaration for Disability Pension

State of Missouri County of Harrison On the 7th day of July, A.D. one thousand eight hundred and ninety, personally appeared before me a clerk of the circuit court, a Court of record within and for the County and State aforesaid, Sampson Brazzell, aged 56 years, a resident of the Township of Union, County of Harrison, State of Missouri who being duly sworn according to law, declares that he is the identical Sampson Brazzell who was enrolled on the 29th day of August, 1862, in Company A, of the 35th Regiment of Missouri Infantry Vol, commanded by Capt. Horace Fitch and was honorably Discharged at Little Rock, Ark, on the 28th day of June, 1865; that his personal description is as follows: age 56 years; height 5 feet 10 inches; complexion dark; hair dark; eyes blue. That he served more than ninety days in the military service of the United States during the late war of the Rebellion and was honorably dischared therefrom. That he is now suffering from physical disability of a permanent character, which is not the result of his own vicious habit, to wit: disease of eyes which impairs the vision, disease of kidneys and back, partial deafness of both ears. Which said disabilities incapacitates him from the performance of manual labor in such a degree as to render him unable to earn a support. That he hereby makes application to be placed on the list of pensioners of the United States, as provided by the Act of Congress approved June 27 1890, entitled “An act granting Pensions to Soldiers and Sailors, who are incapacitated for the performance of manual labor, and providing for pensions to widows, minor children and dependent parents. That he has not received or applied for a pension, not been employed in the military or naval service of the United States otherwise than as above stated. War Department, Record and Pension Division, Feb. 4, 1891. Military Service for Sampson Brazzell; No. 812190. Sir: It is alleged that the above-named man enlisted 19 Aug., 1862, and served as a Pf't in Co A, 35 Reg't Mo. Inf. and was discharged at Little Rock, Ark. on June 28, 1865. The War Department will please furnish an official statement in this case, showing date of enrollment and date and mode of termination of service.

DECLARATION FOR PENSION State of Missouri, County of Harrison On this 23rd day of September, A.D. one thousand nine hundred and eight, personally appeared before me, a notary public within and for the County and State aforesaid, Sampson Brazzell, who, being duly sworn according to law, declares that he is 75 years of age, and a resident of near Eagleville County of Harrison, State of Missouri; and that he is the identical person who was enrolled at Eagleville Mo under the name of Sampson Brazzell on the 29th day of August, 1862 as a Private, in Co. A 35th Regt. Mo. Inf. Vol. in the service of the United States, in the Civil War, and was honorably discharged at Little Rock, Ark on the 28 day of June, 1865. That he was not employed in the military or naval service of the United States otherwise than as stated above. That his personal description at enlistment was as follows: Height, 5 feet 10 inches; complexion, dark; color of eyes, blue; color of hair, dark; that his occupation was farmer; that he was forn Sept 20th, 1822 at Greene County Indiana. That his several places of residence since leaving the service have been as follows: Harrison County, Missouri. U.S. Pension Office Sep 26, 1908 J.C. Barber I, Oscar W. Curry, a notary Public, in and for Harrison County, Missouri, do hereby certify that Sampson Brazzell has this day exhibited to me a pictorial Family Record showing names and dates as follows, towit: Marriages: Sampson Brazzell and Catharine Cook was married Aug 19 1858 NAMES BIRTHS Sampson Brazzell Sept 30th 1833 Catharine Cook Nov 15 1838 Mary A. Brazzell July 1st 1873 I further state that there is no date on said record indicating the time at which said record was made. That there are no alterations or erasures apparent on said record. That the same has ever appearance of being genuine and I would judge from the writing that the same is some twelve or fifteen years old. General Affidavit, State of Missouri, County of Harrison, SS. In the matter of the application for pension of Sampson Brazzell, Co. "A" 35th Reg't Mo.Vol.Inf. Ctf. No. 605471. On this 16 day of November, A.D. 1908 personally appeared before me, a Notary Public in and for the aforesaid County, duly authroized to admisiter oaths Sampson Brazzell, aged 75 years, a resident of near Eagleville in the County of Harrison and State of Missouri whose Post Office address is Eagleville, Missouri. That he is a claimant in the above entitled claim for pension. That there is no public record of his birth in existance so for as he can ascertain and that there is no babtismal record in existance. That the only record of his birth so far as he knows is a family record, made some fourteen or fifteen years ago, and the names and dates on said record were copied by him from an old Bible belonging to Joshua Brazzell, fahter of Claimant, said record is gone from said bible as are a great many other portions of it.

No. 9. State the names and dates of birth of all you children, living or dead. Answer. Check No. 7296007, $39.00 dated Feb 4, 1923, Section 5 returned by ostmaster with information that the above described pensioner died Jan. 4, 1923, has been canceled. E.E. Miller, Disbursing Clerk. The name of the above described pensioner who was last paid at the rate of $30 per month to Jan 4, 1923 has this day been dropped from the roll because of Death
