Reproduced here by kind permission of Philip Riden, Secretary of the Derbyshire Record Society (DRS). The DRS published a full transcript of the "Derbyshire Hearth Tax Assessments 1662-70", edited by David G. Edwards, in 1982, now sadly out of print. I'm very grateful to Sonia Addis-Smith, who kindly provided me with a copy of the relevant section of this transcript.
