My explanation comes from a tombstone of a Vietnamese and is correct. The Fan you refer to is a Mandarin spelling and not Vietnamese.
: Disregard Al Chinn's follow-up. It is simply incorrect. According to the Hundred Families Surnames in both Mandarin Chinese and Vietnamese, if PHAM is spelled with a dot under the A for tone, then the corresponding Mandarin equivalent is FAN with 4th tone. If you want to see the character, go to www.geocities.com/Tokyo/3919/hundred.html ; it is listed at No. 46
