I submit the following report on the sanitary condition of this command during the month designed:

1. Public buildings and grounds, or camps and their surrounding: Two story, frame, sanitary conditions: very good.

2. Drainage, sewage, and disposal of waste: Excellent.

3. Sanitary appliances (filters, sterilizers, incinerators, odorless excavators, etc.): None

4. Water supply: Petersburg, Va. Condition: Excellent

5. Food supply and its preparation: Plentiful and well prepared
a. Quality of the milk: Canned, Carnation Brand
b. Condition of dairies: None

6. Clothing of the men: Fully equipped, Excellent

The C.O. of Remount Depot will turn over Veterinary Hospital except Infirmary pertaining strictly to Depot to the Senior Veterinarian at the Hospital. Joint inspection of Veterinary Hospital will be made by the Officer Commanding Remount Depot and Veterinarian taking over the Hospital and joint report submitted to their respective Chiefs showing the condition of premises. 2. Nearly all of these animals were received from the Artillery Camp at Camp de Souge during the outbreak of Influenza at that place, all had passed through the attack and recovered. During their stay in the hospital all had been subjected to the lime and sulphur treatment many times to obviate any possible spread of Mange. Some cases had ringworm; these were treated for this infection. In addition to the lime and sulphur treatment, no animals were allowed to leave the hospital for evacuation Remount Depot without careful inspection by myself and no animals were evacuated where there was the slightest doubt as to health, or to presence of parasitic skin disease. Many bore indications of having ringworms, but it was not considered economical or to the best interests of the services to hold these animals in the hospital, awaiting a new growth of hair on these spots.

3. It is not the policy of this organization to evacuate an animal to Remount where there is any doubt as to its health, or freedom from parasitic skin diseases. Neither is it our policy to hold animals when they are cured and fit to be evacuated to a Remount Depot.

Subject: Report on Carbon Blanc.

An inspection was made of the Veterinary Hospital, with the following conditions noted.

At the present time there are 1020 animals at this Hospital of which 240 are Mange cases, and approximately 5000 Influenza. Some of the animals have both diseases. There are about 150 animals which are recuperating from Influenza, but which are not ready to be evacuated to the Remount Depot adjoining the Hospital.

The Influenza cases are under stabling, while the Mange cases are tied to lines in the corrals.

As the stabling capacity of the Hospital at present time is less than 500 animals, and as the Hospital will have to evacuate animals from Souge, and Merignac, the present installation is inadequate. In addition there is no provision as present for the quarantining of animals to be received nor isolation for infectious cases. A letter has been forwarded to the C.G. Base Section No. 2 through the Chief Surgeon, recommending that a plot of land adjacent to the Hospital, be acquired for the above mentioned purpose.

Instructions have been given to the C.O. of the Hospital to commence immediately the treatment by hand of the Mange Cases, and to use the Mange solution on all animals in the Hospital after grooming, by reason of their being contacts. The Sulphuration chamber has not been completed, and is not available for use. The Dip in not equipped with a boiler, and in addition is filled with a Cresylic Acid solution.

A number of animals were observed that were hairless by reason of having been stood in the sun after treatment with crude oil. These cases were evacuated from Souge. Efforts will be made to turn the hairless animals over to French farmers until they are ready to be evacuated to the Remount Depot.

As there has been no malleination, directions were given to immediately commence the malleination of all animals, and the same is being carried out.

Directions was given to start numbering the animals, and to keep a record of all cases, which same has not been done heretofore. A number of other directions were given for sanitary and other conditions.

REPORT OF EPIDEMIC OF EQUINE INFLUENZA AMONG ANIMALS AT CAMP DE SOUGE

A. Location, Nature and History of Epidemic: On May 10th, 1918, Captain Alan M. Chesney, M.C., of the Central Medical Department Laboratory was detailed to make a bacteriologic investigation of the Epidemic of Influenza among horses at Camp de Souge. The following condensed abstract from his excellent report (submitted June 4) will give the salient features of this epidemic at the time of his investigation.

1. Location: There were three (3) Remount Depots in Base Section No. 2, but the epidemic was then confined to the horses stabled in one of these depots, located at Souge Camp. To this Depot have been brought only French horses, whereas the animals from America were kept at the other two Depots at Carbon Blanc and Merignac and were said to have been free from the disease.

2. Nature of Epidemic: Two diseases were occurring in epidemic form at Souge Camp Remount Station.

a. Strangles of Coryza contagiosa equorum.
b. Influenza, known also as stable fever or shipping fever.

Strangles, is characterized by an acute inflammatory process of streptococcus origin in involving the nasal mucosa and submaxillary lymph glands. This disease, though infections and widely prevalent, is relatively benign as compared with influenza, and has been responsible for many deaths.

Influenza is a highly infectious disease characterized by a variety of clinical manifestations. At Souge the pulmonary form has been responsible for the majority of deaths. The cause in believed to be a filterable virus the investigations in the past as well as those of Capt. Chesney have shown the presence of streptococci and gram-negative bacilli in the lungs as well as in the blood stream. It has been generally believed that these organisms are secondary invaders.

Strangles and Influenza are frequently found in association and both are spread chiefly by contact, after the manner of droplet infections.

3. History of Epidemic: The Remount Depot at Camp de Souge was opened on March 28, 1918. Large numbers of horses were admitted after that time. The date of the first recorded death was April 2nd, the diagnosis being Equine pleuropneumonia. From that time on, deaths from this disease have been of daily and increasing occurrence. An accurate mortality rate cannot be estimated; approximately 10 percent of all animals received at the Remount Station at Camp de Souge during April and May were lost through influenza or one of its complications, such as pleuropneumonia or purpurahemmorrhagica

B. Subsequent Course of Epidemic: Throughout the summer months the epidemic continued. Accurate data are not available but receipts for dead animals sold by the Depot Quartmaster show a strikingly increasing number:

It may be assumed that 95 percent of the deaths were due to some form of the epidemic.

During the first part of August the Remount Station at Camp de Souge was discontinued though no official notifications of this fact has been received at Camp Headquarters. As many sick horses as could be evacuated (about 1100) were moved to Veterinary Hospital No. 4, Carbon Blanc. On August 19th Capt. Wm. L. Moss, M.C., Chief Epidemicalogist for Base Section No. 2, investigated the situation at Camp de Souge. The conditions he found were as follows:

There were 7,400 horses and mules in the camp. At least 25 percent of those were sick. Sick horses were being worked side by side with healthy ones. Animals, according to the statement of Col. Healy, the Camp Commander, were being admitted to the camp directly from the R.R. station after two to six days crowding in boxcars with insufficient food and water. Many of these were sick on arrival and died shortly after admission to the camp. Without quarantine or detention these animals were being issued to outgoing brigades. No effective attempt had been made to isolate infected horses or mules. The stables were found in a dirty and much neglected conditions. Mangers and hayracks were caked with black incrustations formed by dust and deposits of nasal and buccal secretions. Stable floors were pawed away so that animals stood with their fore feet in a hollow, which necessitated stretching to reach their feed. In many case sick animals were to weak to do this.

A plan was drawn up in the Base Surgeon's Office to remedy the situation:

1. Quarantine of all incoming animals for ten days for the purpose of discovering and the removing all infected ones.
2. Immediate to examination of all animals in the camp for purpose of discovering and removing all infected animals.
3. Daily inspection thereafter for same purpose.
4. Establishment of hospital for isolations of sick animals, those suffering with each disease to be segregated separately.
5. Establishment of a detention hospital for animals sick but not yet diagnosed.
6. Through cleaning and disinfection of all stables and corrals.
On August 28th, Major L. English, M.C., of the 60th C.A.C., was detailed to take complete charge of handling the epidemic as representative of the Chief Surgeon, Base Section No. 2, S.O.S., A.E.F. He continued his duties until September 1st, during which time he accomplished a most striking improvement in the general situation. Because of the threatened departure of the 60th C.A.C., Major English was relieved of his duties and 1st. Lt. C.A.L. Binger, M.C., was ordered by Verbal Order, Chief Surgeon, Base Section No. 2, S.O.S., A.E.F., to substitute for him.
C. Conditions on September 1st.
At this time the following situation obtained:

1. Statistics:

Horse and Mules in camp - 5551
No. of sick animals - 2357
Percentage sick - 42.4
No. of deaths and destroyed on proceeding day - 26
No. of sick in Veterinary Hospital - 940
No. of sick scattered throughout the camp - 1417

2. Hospital facilities
Was inadequate consisting in all of 12 stables with a capacity of about 450. The 52nd Brigade was evacuating and turning over to the hospital great numbers of weak quarantined infected animals for which there was insufficient stable room and personnel. The animals were being picketed out in the hot sand in close approximation. No classification or segregation was possible under the circumstances.

3. Camp Stables
Throughout the rest of the Camp were scattered sick animals, isolated from well ones as best as stable conditions would permit.

D. Plan and Organization:

1. Plan:
a. To increase hospital capacity thus permitting removal of as many infected animals as possible from rest of camp stables.
b. To classify animals in Hospital according to stage of disease:

Influenza
Pneumonias
Surgical cases
Convalescents

c. To isolate those animals for which there was no room in hospitals.

d. To provide for thorough clean up of camp stables

Organization:
The following organization was immediately adopted:

E. Execution of Plan

a. Increase of Hospital Capacity:

1. Addition of 30 stables evacuated by 52 nd Brigade

2. Plans submitted to Camp Engineers for construction of three corrals with capacity of about 700 head. First corral completed and in use September 8th. Third corral completed and in use September 14th.

3. Erection of numerous picket lines

b.Classification of Animals
1. Seriously sick animals were put in stables of Group A one stable being reserved for cases of pneumonia.
2. Mildly sick animals were put in stables of Group C.
3. Convalescent animals were put in stables of Group B whence they were removed to the convalescent corrals. (Each group of stables was in charge of a cavalry officer with his own troop under him. Owing to insufficient personnel it was impossible to give the horses all the attention they needed, such as daily grooming. The Troops were kept busy hauling forage, watering animals, cleaning stables and moving animals for purposes of classification.
4. Surgical cases were moved to stables E and D in the northeasterly section of the camp, where they were excellently cared for by Lt. Ridley and his Veterinary Corps men from Veterinary Hospital No.4.

c. Isolation of Animals not in hospital
As far as possible all sick animals were moved into the hospital. The sick of outgoing Brigades were taken in first. On September 1st there were 1417 sick animals scattered through the camp. On September 14th there were less than 200. Adequate provision was made for their isolation by setting aside stables and troughs for their use. As they were nearly all convalescent it was deemed wiser to leave them to the care of the Regimental Veterinarians than to crowd the hospital still more.

d. Clean-up A meeting of Camp and Regimental Veterinarians was called. G.O. No. 62 was read aloud and each provision explained and discussed. Supplies were issued from the Veterinary Supply Depot organized at the Hospital. Daily inspections were instituted. The progress of the clean-up work is indicated on the map. Stables marked clean have been inspected by the Camp Veterinary and the Medical Officer in charge.

F. Results: These are best shown by Morning Reports and the graphical chart.

1. Strengths:
The distribution of horses among the Brigades is constantly shifting. An outgoing Brigade will turn into the hospital all its sick animals and replenish partly from other Brigades, partly from new animals brought into camp. This makes any effective isolation between Brigades impossible and it may be safely said that all animals in the Camp have been exposed to the infection. It is to be anticipated that this disease will accompany the Brigades which have recently left the Camp, even though at the time of departure their strength was composed of apparently well animals.

2. Sick:
Reduction of approximately 8 percent, in this category are included all stages of the disease. Many are convalescent (about 500) and will soon be ready to transfer to a Remount Station. A certain percentage will inevitably die.

3. Deaths:
Reduction approximately 50 percent, one cannot expect an abrupt drop in the number of deaths. With the disease once present the death rate may be from 5 to 10 percent, The average weekly deaths should from this time on gradually diminish.

G. Recommendations for Further Management of Epidemic and Prevention of Recurrences:

It is important for the Camp Hospital Veterinarians to have command of their own personnel. The present system of having Cavalry and Remount troops under line officers (often of superior rank) do the necessary work for the Veterinary Officers is awkward and inefficient.

2. Morning Reports to Camp Headquarters
These should be indefinitely continued, supplying information on strength, number of sick, variety of infectious diseases, number and causes of deaths. Morning Reports should not be consolidated for whole Brigades, but figures supplied by each individual organization. Consolidated Brigade reports are necessarily inaccurate because of the constant exchange of animals form one Regiment to another. The Regimental Veterinarian should be responsible for the accuracy of the Morning Report of the animals of his Regiment. The Regimental Adjutant should be responsible for sending the report to Camp Adjutant. The Camp Veterinarian should get required data from Camp Adjutants Office.

3. Sanitary measures: - to be enforced and supervised by the Camp Veterinarian
a. New Horses: All horses entering camp should be issued from a Remount Station when they have been treated according to Instruction Circular No. 1, Remount Division, June a, 1918 This obviates the necessity of detention or quarantine in camp.
b. Sick horses: Animal exhibiting first sign of infectious disease should be immedia strangles or influenza, immediate contacts in same stable should be isolated for the next ten days. During this time they should not be allowed to water from common trough. They should be marked as suspects.
c. Overcrowding: Should be avoided as much as possible. Where practicable the open corral should be used. The horses will soon be exposed to the rigors of the front and this may serve to harden them as well as to prevent spread of communicable diseases.
d. Cleanliness: The following program is the minimum of care to be required of all regiments:
1. Daily cleaning: All water troughs to be scrubbed thoroughly with stiff brush at least once each day and allowed to dry in the sun. Constant removal of manure in stables and corrals and raking of floors to keep them level.

2. Weekly cleaning (under supervision of commissioned officer). Horses to be removed from stable and picketed out. Manger and feed racks to be thoroughly scrubbed with soap and hot water.

3. Monthly cleaning (under supervision of commissioned officer) Once each month all stables to have a thorough cleaning. Mangers to be soaked with lye solution then scraped and finally scrubbed with soap and hot water. Hay racks to be taken down and treated similarly. Walls to be scrubbed with lye solution. The use of sprays and whitewash should be left to option of the individual organization. White wash should not be used on or above mangers.

It is important that this work should be assigned to an incoming rather than an outgoing Brigade. Departing Brigades are too preoccupied to do police work with any enthusiasm or effectiveness. The incoming Brigade usually precedes its animals. The Farriers and Stablemen can be effectively used immediately upon their arrival by preparing the stables for their animals. A motorized Brigade should clean stables in similar manner as a horse drawn one.

H. Conclusion of Report: This Report would be incomplete without mention of the persistent and excellent work of the three Veterinary Officers:

Subject: Mallein Re-action

1. As matter of explanation of weekly report No. 102, December 7, 1918, relative to glanders, wish to state the following facts:

a. Out of thirteen (13) destroyed today, eleven were received from Veterinary Hospital No. at Nevers, France. Eight of these were received October 18, 1918 and three, October 30, 1918. Two were animals brought from Camp Souge at the time Veterinary Hospital No. 4 moved to Carbon Blanc. These cases were all treated at time of entrance and failed to re-act.

b. One case that would not re-act to Mallein was as fine a case of clinical glanders as I have ever seen. Characteristic nasal discharge, pneumonic symptoms, ulcerous nasal septum and subcutaneous nodular formation over body, was in abundance. Maxillary glands were also enlarged and nodular. It certainly was a case of some standing. It seems that the mallein made an acute case of it, although no re-action whatever was noted about eyes, either on first, or second test.

c. All cases were injected December 5, 1918 and read December 6, 1918. Doubtful, as well as positive and clinical cases, were retested December 8, 1918. Today, December 9, 1918, we felt justified in destroying the thirteen cases, which had been handled with great care. They were placed seven feet under ground.

d. Every precaution has been taken to avert further trouble. Vigorous disinfection will continue and it is our belief, that we can soon dispel all danger.

2. Up until this time, we have had but one re-action to mallein, and it was a closed case, that re-acted slightly only. This animal was destroyed and reported September 14, 1918 to your office.

3. It is our firm belief that glanders entered our hospital October 18, 1918, although there was not the slightest re-action at time of test.
