* The obituary for Jack S. Crowder says his survivors included
his wife Betty (Miller) Crowder; one son, Daniel Dove; and two daughters,
Jacklyn Miller-Alumbaugh and Carolyn Miller-Collins. Because of the hyphenated last names of the daughters, it is
possible that he might not have been their birth father.
** Some of the
mentioned “brothers” and “sisters” in Jacklyn's obituary were listed in the
1949 obituary as the children of her grandfather Jesse Miller and his wife
Violet Myers Miller. The Betty listed
in that obituary below is Jacklyn’s mother.

LAPORTE, IND. — A factory boiler used to cure rubber seals
and gaskets exploded Thursday night, killing two workers and injuring six, the
plant's owner said Friday. Jeffrey Burnel, owner of the American Rubber
Products Corp., said federal officials are searching for reasons the
20-year-old gas-fired boiler exploded. The plant, which has 250 employees, will
be closed until at least the middle of next week.

At approximately 9:46 a.m. on
March 4, 1993, an explosion occurred in the boiler room at American Rubber Products
Corporation (ARPC) of Laporte, IN. Thirty employees were in the plant at the
time of the explosion. This explosion was caused when a York Shipley gas-fired
boiler, serial #7438, exploded without any warning and without any unusual
conditions apparent. The blast sent a heavy, 6 foot diameter, boiler end plate
through a wall and into the adjacent press department. Escaping scalding water
and steam followed the plate into the employees' work area. At the same time,
in the opposite direction, the large boiler was propelled through a concrete
and brick wall before resting 50 feet outside the building. On the way out, the
boiler created a 15 square foot hole in the outside wall. No fire occurred
after the incident. Employees #1 through #4 were killed. Employees #5 through
#8 were hospitalized and Employees #9 through #14 were treated and released
from the nearby hospital emergency room.

Cunningham, Christopher L. Cunningham

[nwitimes.com]; by WILLIAM LAZARUS, March 28, 1994 [WRITTEN ABOUT 1993
ACCIDENT]
One year later. American
Rubber workers still feel pain
LaPORTE - A huge boiler exploded at American Rubber
Products Corp. a year ago in one of Indiana's two worst industrial accidents in
a decade.
Four workers were killed and 10 injured the night of March
4, 1993, at the firm's LaPorte plant. The blast catapulted a 40,000-pound
boiler, tumbling its pressure vessel end over end to stop 20 feet outside the
plant.
Time has not healed all wounds. Some workers and their families believe American Rubber is at
fault for
failing to maintain its
boiler, a claim disputed by the company. They express anger at the $3,625 fine
imposed by the Indiana Occupational Health and Safety Administration. "I
think it's outrageous, man. I could have paid that," said Robert White,
whose sister was decapitated.
American Rubber employs 450 workers at its central LaPorte
plant and three satellite operations in other states. The company sells $20
million of automotive rubber parts yearly.
"Four people got killed, and they get fined
$3,000," White said. "I tell you, that's ludicrous. But what can you
do about it?"
"We could never recoup in penalties for the loss of
life," responded IOSHA
Deputy Commissioner David Bear. "The penalties are set
according to a federally adopted formula that has to be applied. They are not
written for fatalities or catastrophes. They are written for violations of the
law."
Jeffrey Bernel, chief executive officer of American Rubber,
expressed a similar view, saying, "Would a million dollars bring back
those people?" He complimented IOSHA for its work, saying the agency had
suggested psychological
counseling for employees and
"tried to help us determine what the hell happened here."
Lawsuits in the wake of the accident also may help in that
determination. But American Rubber is
unlikely to be named as a defendant because workers compensation, while
providing automatic medical coverage and funeral expenses, bars lawsuits
against employers except in cases of intentional wrongdoing.
Differing accounts
An IOSHA worksheet filled out after the accident notes no
willful or repeated violations. It also states, without elaboration, that
"the boiler had to be replaced."
Bernel said that statement refers only to the need to get a
new boiler after the accident. "It was inspected by a state inspector every
year," he added. "If they felt it had to be replaced, we wouldn't
have been able to use it."
But White said his sister Linda at times "hated to go
to work. She would say, 'The boiler went crazy last night. It's going to
explode one day.' ... She said it made funny noises (and) vibrated. She always
said it's going to kill somebody, but I guess she never thought it was going to
kill her."
One current American Rubber worker said the boiler long had
rumbled and shaken the building, and that a temporary maintenance worker had
been complaining that it was dangerous two weeks before it exploded. But two employees denied any unusual shaking
or noises occurred, and one former employee doubted Linda White had been afraid
of the boiler. Boilers "rumble
from time to time," Bernel said. "The boiler had been acting normally
for years and years ... There was nothing that we as management or maintenance
felt was unusual ... What happened was an aberration, one in a million."
A post-accident inspection report by an insurer states
"the boiler exploded without warning and without any unusual conditions
apparent." But it also notes that dust caused vibration problems with part
of the boiler and cites a pre-accident failure or removal of a key support
piece.
A settlement agreement between American Rubber and IOSHA
states that the company's contract service failed to provide proper maintenance
of the boiler.
"IOSHA felt and we felt there was an improper
installation done when we added a new burner five years ago," Bernel said,
adding that the installation was performed under the direction of the contract
service.
Whoever may be responsible, a number of American Rubber
employees asked not to be named. They said the firm had advised them to refer
inquiries about the boiler and the accident to management.
Bernel said employees could speak for themselves but that
only he could speak on behalf of the company. He said he is responsible for
gathering all the facts, adding that release of bits and pieces "creates
rumor, innuendo and falsehoods."
A painful episode
Sue Ellen Johnson finds it difficult to discuss the
explosion that caused her to lose a hand, her eyesight and her kidneys. "I don't remember any of it,"
she said. "I was in a coma for four months. All I know is that it was a
very tragic thing."
Jill Johnson said workers compensation paid her mother's
medical bills, including the ongoing expense of four hours of dialysis three
days a week. But Jill recently had to leave her job as a nurse's assistant to
care for her mom without pay. "She forgets a lot of things" like
taking pills and turning off the oven, Jill explained. "That's basically
why I'm here - to be her memory."
For Gayla Holt, it's the memories that are painful. "I
heard a loud noise. I turned, and all I saw was a cloud of smoke coming at me
... I thought it was a press," Holt said. "All I could think of was I
was going to die in a stupid factory." Holt recalled that a friend, Vicki
Dirks, "came out of the smoky haze. She was all wet, covered with blood,
walking to us, begging us to help." After Dirks was in an ambulance, Holt
said that another friend, whose skin had been burned "right off her
hands," just "lunged into my arms. She said, 'Please take care of my
little boy for me.'"
A third friend, Holt said, was bleeding profusely. She
called over
policeman to help, but,
"He said, 'Do you have rubber gloves?' ... "The back of her head was
split open and he wouldn't touch her because we didn't have rubber
gloves." Holt said it still makes
her angry to think about it.
"That's the first time I ever heard that come up,"
said LaPorte Deputy Police Chief Walter Brath. "I know I didn't have
rubber gloves on. We weren't worried" about that at the time. Four
separate police departments responded to the scene.
Some officers, Brath said,
took heroic, though questionable, risks in quickly entering the building
without first being able to determine the dangers.
Mental horrors
Holt recalled that
just five minutes before the explosion, she had been sitting at break with
Linda White and Joan Erickson, who were killed instantly while working presses
just 10 feet away from the boiler.
"I had been throwing snowballs at them all night. I'd grab a
snowball, run by them and throw. It was just goofing around," Holt said.
Linda was 38 years old. Joan was 18.
Holt doubted that Linda was overly fearful of the boiler. "She
liked the area she worked at. If she was really scared, she wouldn't have
wanted it," Holt said, adding,
"At least that's how I feel."
After the accident, Holt said, she herself was haunted by fear. At work, "I kept jumping at every
little noise. I felt really on edge. I always had this feeling that something
bad would happen to me just out of the blue," Holt said. She added that
she quit in June, after the company started planning to reopen the press room
in which her friends were killed. Even
now, Holt said she can't watch violent TV shows, and sometimes "I just have
the feeling something is going to come along and whack my head off, just like
it did hers."
Soliciting the dead?
Chris Cunningham, 20, was working at American Rubber
as temporary help, hoping to get a permanent job after a couple of weeks, said
Terry Harmon, who said he had helped raise the young man for several years.
Cunningham, he said, suffered severe burns over much of his body, and died
after four days in the hospital and about $200,000 in medical bills, which were
paid by workers compensation. "They said if he did live, he would probably
be a vegetable," Harmon said.
After the accident, he said, Chris got a letter from the Washington,
D.C., law firm of Coale, Allen and Van Susteren, which specializes in plant
explosions. "We got to talking" and decided to hire the firm, Harmon
added. Lawyer Phil Allen said that
Cunningham likely wasn't dead when the firm sent its letter, and that, in any
event, such solicitations are allowed under a U.S. Supreme Court decision.
"We're suing the inspectors and installers and sellers
of various pieces of the boiler" on behalf of two workers who were injured
and three who died, including Cunningham, Allen said, describing the lawsuit
filed in Indianapolis. "The
theories are negligent inspection and improper design, manufacturing and
installation."
Since some of the workers injured or killed in the
explosion were temporaries hired through Manpower, it's possible that the
workers compensation would not bar lawsuits by those individuals against the
employer, Allen said. "We're still
working on the discovery and investigation," he added. "We haven't
made a decision" whether to sue American Rubber on behalf of temporary
employees, adding that, thus far, accounts as to American Rubber's
responsibility have varied widely.
A deterrence issue
Unlike Indiana, some states allow employees covered by
workers compensation to sue their employer in cases involving gross negligence
by the employer. "That puts the
responsibility on the shoulders of those people who have
caused the wrong" and
makes it more likely that employers will pay close attention to safety, Allen
said. He said OSHA fines and workers compensation payments are usually are too
meager to fulfill that deterrence role.
Locally, the son of Linda White retained Merrillville lawyer
Peter Benjamin, according to his uncle Robert White. Benjamin's partner, Ted
Leonas, said he is largely handling the case because Benjamin is busy with his
duties as Lake
County assessor as well as
his campaign to be elected sheriff.
Leonas said Benjamin & Leonas will file a lawsuit
against parties responsible for making and maintaining the boiler. Citing an
insurance company inspection report adopted as an official report by the state
of Indiana, Leonas
maintained that "a case
for gross negligence" might be made against American Rubber but for the
workers compensation bar.
Were parts altered?
In its report, the Hartford Steam Boiler Inspection and
Insurance Co. found that bolts holding a burner assembly had worked loose,
causing the burner to tilt, and eventually resulting in a rupture. "The installation of the burner in 1988
was not made without having to alter parts of the boiler front cover plate and
the burner assembly," the report states. It also notes, "Any
supporting device for the burner assembly was either removed or had failed in
some manner between 1988 and 1993."
In addition, the report notes that mercury switches for the
pump and another function appeared "in satisfactory condition" but
"the wires had either been ripped out by the occurrence or had been
removed" prior to the explosion.
Bernel asserted that no wires
had been ripped out of the safety switch, and that the condition of the switch
was not related to the explosion. In any case, he said the company's contract
service would have been responsible for any
improper installation or
maintenance.
IOSHA initially assessed a $5,000 fine against American
Rubber for improper maintenance of its 18-foot-high, 7.5-foot-wide boiler. But
in a settlement, the agency reduced that fine to $2,500, and noted only that
the company's "contract
service" had failed to
provide proper maintenance. A separate fine for an unrelated safety violation
accounts for the remainder of the $3,625 penalty. The fine "is outrageous," Leonas said, adding, "I
think that people are being fined equal dollars and, in fact, in many cases
paying more dollars for very, very minor, trivial infractions."
Ken Haselwander, a spokesman for IOSHA, said repeat
violations, which were not found in this case, can cause fines to rise
dramatically, with a $10,000 penalty proposed for each such violation. Since the accident, he said, American
Rubber's 280-employee plant at LaPorte has not been reinspected because the
normal inspection cycle has not come around and no workers have complained to
IOSHA about safety conditions.

Ray M. Stanley: Stanley
retired from Air Force
Ray M. Stanley retired after 20 years in the Air Force and
then worked in civil service for 20 years.
He was a devoted family man, said his son-in-law, Vance G.
Safley of San Antonio.
Stanley, 60, died Thursday at a local hospital from thyroid
cancer.
Born in Parkersburg, W.Va., he attended San Antonio College
and Southwest Texas State University, where he received bachelor's and master's
degrees.
Stanley married his high school sweetheart, Ava Lee
Doughtery, on July 31, 1952. They had three children.
"He was a very devoted family man who, during his
early career, worked multiple jobs to provide for his family," Safley
said.
Stanley and his family often moved during his military
career but settled in San Antonio in 1967, five years before he retired as a
technical sergeant. He then started his civil service career at Lackland AFB.
Most of his civil service work was at Lackland AFB, with
the exception of three years at Randolph AFB, said his daughter, Cheryl
Stanley.
Stanley held several jobs in the technical training
division at Lackland and also was chief of the operations division. He was a
career program administrator at Randolph.
"He touched many lives in his civil career," said
a former co-worker, Rose Sullivan of San Antonio."He was a mentor to a
great number of people."
Stanley also had a reputation for fairness, his daughter
said. "He treated people fairly in times when they weren't being treated
fair."
After he retired in 1992, Stanley devoted much of his time
to landscaping projects, his daughter said.
"He had quite the green thumb," she said.
"He would plant acorns, and now his yard is full of oak trees. He would
watch trees grow and then cut them down to plant others," she said.
Stanley used the wood from the trees to pursue another
interest woodworking, his daughter said. He carved belt buckles, she said.
He had a keen wit, his daughter said. "He always was
one to inject humor in disagreements and make us think that what we were
arguing about wasn't that important, that things could be worse."
Stanley also enjoyed the outdoors and would take his family
on camping trips, Safley said.
"I wouldn't call him a mountain man, but he very much
enjoyed the outdoors and camping. He had a trailer that the family would go
in," he said.
Additional survivors include his wife; another daughter,
Cynthia Stanley of Winston-Salem, N.C.; a son, Jeffrey Stanley of San Antonio;
a sister, June Miles of Jacksonville, Fla.; a brother, Alan Stanley of
Charleston, S.C.; and a grandson.
Funeral services were scheduled for 10 a.m. Tuesday in
Chapel 3 at Lackland with the Rev. Sammie Swim officiating. Burial will follow
in Fort Sam Houston National Cemetery.

Submitter's Notes: My copy of this obituary was of terrible
quality. Throughout the entire obituary
each letter was surrounded with tiny dots.
Using google to ascertain names of places and businesses, I think most
of my translation here is correct.
However, names with an asterisk and dates with an asterisk are only my
best guess. Some words I could not
figure out at all.
