Smith, Clerk; the same being a Court of Record of the County and State aforesaid Stephen Taylor a resident of Clendenin County of Kanawha State of West Virginia who being by me duly sworn according to law, on his solemn oath, deposes as follows, to wit:
"I am the identical Stephen Taylor who was enrolled on the 29" day of February 1862 in Company G of the 9" Reg't of West Virginia Vol's., commanded by Captain Robert Laughlin and I was honorably discharged at Cumberland, Va on the 21" day of February 1865 and my age is now 42 years. While in the service aforesaid, and in the line of my duty I received the following disability, to wit: At the action at Carter's Farm, Va. July 20, 1864, while in a charge, I received a shot across the side of my head which injured my hearing, and I have never recovered it. Also while at work in building the quarters at Cumberland, Md. I was severely hurt in my back, and which injury has ever since so weakened me that I can do no hard work. The regimental surgeon treated me at both Carter's Farm and at Cumberland. I claim a pension for injury to my back and loss of hearing.
I have never been employed in the Military or Naval Service of the United States otherwise than set forth above. Since leaving the service, I have resided at Clendenin, W. Virginia and my occupation has been farming before my entry into the Service aforesaid I was of good, sound physical health, being at enrollment a farmer and I am now much disabled from obtaining my subsistence by manual labor by reason of my disabilities above stated, received in the service of the United States, and I make this Declaration for the purpose of being placed on the Invalid Pension Roll of the United States. I hereby appoint and empower, with full power of substitution, Nathan W. Fitzgerald, of Washingtonn City, D. C/ my true and lawful Attorney to prosecute my claim.
