I have the honor to request that Sergt. John Hamill Co. "D" 91st Pa Vet. Vols be discharged to enable him to be mustered as 1st Lieut. of said Company he having received a Commission from the Governor of Pennsylvania Sept 6th 1864 dated July 26th 1864

This Company has Fifty-two (42) [note: the 'Fifty' and '4' are obviously overwritten; I can't tell from the microfilm which is the later; it looks like 'Fifty-two' and '42', but that is obviously wrong.] men present and a total of Present and Absent of Sixty Eight (68) with but One Commissioned Officer namely Capt. Wm H. Carpenter
