I have the honor to request that 1st Sergt Wm H Fraily Co H 91st Penna Vet Vols be discharged to enable him to be mustered as 1st Lieut of said Co. he having recd a Commission from the Governor of Pennsylvania
This Company has twenty nine enlisted men present and a total of present & absent of forty two.

Charge: Disobedience of Orders
Specification: In this: that said Thomas F Walter 1 Lieut Co "A" 91st P.V.V. having been duly detailed to go on Picket by the Adjutant on the 31st day of December 1864 And having been told by Lt. Col Eli G Sellers Commdg the Regiment that he could not be excused unless by Surgeons Certificate did Positively refuse to obey the order stating that he would go under arrest first
All this at the Camp of the 91st Pa V Vols on or about the 31st day of December 1864
