VOLUNTEER ENLISTMENT

I, Robert Ellingsworth, born in Essex Cy in the State of Delaware aged fourty-four years, and by occupation a Butcher DO HEREBY ACKNOWLEDGE to have volunteered this twenty-fourth day of December, 1863, to serve as a Soldier in the Army of the United States of America, for the period of THREE YEARS, unless sooner discharged by proper authority; Do also agree to accept such bounty, pay, rations, and clothing, as are, or may be, established by law for volunteers. And I, Robert Ellingsworth do solemnly swear, that I will bear true faith and allegiance to the United States of America, and that I will serve them honestly and faithfully against all their enemies or opposers whomsoever; and that I will observe and obey the orders of the President of the United States, and the orders of the officers appointed over me, according to the Rules and Articles of War.

I certify, on honor, that Robert Ellingsworth, a Private of Levit T.A. Hope Company (E) of the 91st Regiment of Pennsylvania VOLUNTEERS of the State of Pennsylvania, born in Essex County, State of Delaware, age 46 years; 5 feet 6" inches high; Dark complexion, Brown eyes, Dark hair, and by occupation a Butcher, having joined the company on its original organization in Philadelphia, and enrolled in it at the muster into the service of the United States at Philadelphia, on the 20th day of August 1861, (or was mustered as Veteran Vols, by Levit Swan at Bealton, Va on the 26th day of December 1863, to service in the Regiment), for the term of three years and having served HONESTLY and FAITHFULLY with his Company to the present date, is now entitled to a DISCHARGE by reason of Died Sept. 22nd, 1864 from wounds received in action in front of Petersburg, Va June 18th 1864.

The said Robert Ellingsworth was last paid by Paymaster M. Holman to include the 26th of December 1863, and has pay due him from that time to the date of death. He has received $60 Bounty, advanced by the United States on account of clothing and One Month Pay in advance.

Given in duplicate, at The Pegrem House, this 12th day of October 1864.
