Civil War Pension Application, July 30, 1890

What follows is the application for a Civil War pension by Samuel Henry Perkins. This comes from a form that either he or someone else filled out and turned in to the court. I've colored the text that he filled in to give you an idea what the form looked like.
 



 

DECLARATION FOR ORIGINAL INVALID PENSION.

To be executed before a court of record or some officer thereof having custody of its seal.

On this 30th day of July, A.D. one thousand eight hundred and ninety personally appeared before me, J.N. [Gumer?] of the Judge of Probate, a court of record within and for the county and State aforesaid, Samuel H. Perkins, aged 70 years, a resident of the _____________ of Cave Hill county of Cedar, State of Missouri, who, being duly sworn according to law, declares that he is the identical of Samuel H. Perkins, who was ENROLLED on the 27th day of March, 1862, in company "E." of the 8th regiment of M.S.M. Cav. Vols. commanded by ____________, and was honorably DISCHARGED at Lebanon, Mo. on the 15th day of April, 1863; that his personal description is as follows: Age, 70 years; height, 5 feet 9 inches; complexion, Fair; hair, Dark; eyes, Blue. That while a member of the organization aforesaid, in the service and in the line of his duty at near Linn Creek in the State of Missouri on or about the _______ day of April or May, 1862, he incurred right Inquinal rupture, while drilling by being thrown upon the horn of his saddle by the rearing and [???] of his horse. This soon resulted in Double Inquinal Hernia. He also contracted Chronic Diarrhea while in camp at Jefferson City, Mo. in Aug. 1862.

That he was treated in hospitals as follows: was treated by Post Surgeon at Jefferson City, Mo.

That he has not been employed in the military or naval service otherwise than as stated above _______________

That since leaving the service this applicant has resided in the county of Cedar in the State of Missouri, and his occupation has been that of a Farmer. That prior to his entry into the service above named he was a man of good, sound physical health, being when enrolled a Farmer. That he is now Wholly disabled from obtaining his subsistence by manual labor by reason of his injuries, above described, received in the service of the United States; and he therefore makes this declaration for the purpose of being placed on the invalid pension-roll of the United States.


NOTES: The application was eventually approved; he filled out a Bureau of Pensions questionaire in 1898 that referred to his next quarterly payment. The questionaire asked if he was married and if so who his wife was (with maiden name); when, where and by whom he was married; what record of marriage exists; was he previously married and if so who was his wife, and what was the place of her death or divorce; and the names and dates of birth of any living children. What this information was used for I do not know.

His discharge certificate notes that he was discharged "by reason of special order No. 69 Headquarters state of Missouri Adjutant General Office, Mo., April 15, 1863."

Samuel Henry Perkins was a private of Captain Henry McCabe's Company E, 8th Regiment of Missouri State Militia Cavalry Volunteers.
