Age 28 years, married, 3 children. Born Sept. 25th, 1871, Castalia, OH. Resides at Rochester, Ind. In case of injury notify wife and H.S. Drake (father) at Rochester. Formerly employed 9 months on the Pan Handle, Chicago Division. Enters the service of the C&E Chicago Division at 9 a.m., Nov. 3rd, 1899. His attention called particularly to Rules 202 to 208 and 228.

Dismissed to reduce the force April 28th, 1900. Reemployed Sept. 4th, 1900 as a brakeman on the Chicago Division. Discharged July 6th, 1901 to reduce the force. Reemployed July 22nd, 1901 as brakeman, Chicago Division.
