Age 23, single, residence Huntington, Ind. Formerly with NY,NH&H Ry 3 years as brakeman and extra conductor. Refers to W.J. Crosby, Supt., Harlem River Yard, New York City. Recommended by Conductor Moak. Entered service of the C&E as brakeman, Western District, Sept. 5th, 1891.

Suspended 5 days for damage to a switch at Chicago June 22nd, 1892. Suspended 5 days from Oct. 12th for damage to switch at North Judson, Oct. 3rd, 1892. Suspended 10 days from Nov. 1st for refusing to brake on a train of freight from Hoovers to Huntington, oct. 29, 1892, because engine had a pilot on each end. Reprimanded in derailment of engine 3 at Palmer, Jan. 26th, 1893.

Suspended 10 days from July 14th, 1893 for failure to accompany his engine with one car to coal dock at North Judson, in backing up they backed off derail. Granted 30 days leave of absence from July 25th, 1893.

Suspended 5 days for not moving at a livlier speed when sent by his conductor to flag No. 5 on whose time they were switching at Hammond on 2nd 83, March 3, 1894.

Promoted to extra conductor, Nov. 12, 1899. Sept. 24, 1902, dismissed, see discipline book.
