Horrocks from any and all claims, liabilities, judgments, and proceedings, both at law and in equity, arising from my participation in the LeStrange/Strange/Strang/Stronge/Strong Surname DNA Study and Project, and further do hereby consent to the study coordinator(s) publishing the y-DNA results and inserting any other results, such as pedigree/ancestor information on the internet or in any media.
The undersigned acknowledges that he or she has read the Legal Notice at the bottom of this page. The legal notice is not a part of the form.
This release shall be binding upon and inure to the benefit of the parties, their successors, assigns and personal representatives.
