Jamestown, Grant Co. Wis. were by one united in Marriage at Fairplay, Grant Co. Wis. according to the laws of the State of   This certificate that on the 18th day of February, 1864, Lot H. Carley of Jamestown, Grant co. Wis. and Margaret A. Daigh ofWisconsin.

Sir:

I have the honor to return herewith your request for a report of hospital treatment in Claim No. 341726, with such information as is furnished by the records filed in this Office, via: that Lott H. Carley, 1st Sergt. Co. E 5th Iowa Cav. was admitted to Div. No 1 G.H. Annapolis, Md. Nov. 26, 1864 with cortritus and furloughed Dec. 17, 1864.

Before me a County Clerk in and for the aforesaid County and State duly authorized to administer oaths, Personally appeared, James L. Saddler, to me well known to be reputable and entitled to credit; who being sworn according to law, defuses as follows:I have been personally acquainted with Lot H. Carley, applicant for invalid Pension since 1854. He was a sound and able bodied man at and prior to his enlistment. He returned home immediately after his Discharge in the year 1865 in very poor health, being troubled with scurvy and diarrhea. He was treated first by Dr. Harney, and later by Dr. McBrien, both of Grant County, Wisconsin. Dr. McBrien died about three years ago or about the year 1876. Dr. Harney is serving out a life sentence in Prison at Waufan, Wisconsin. The applicant has not had good health since his return from the army. And deferent further states that his knowledge of the facts above stated was derived from his acquaintance with the claimant, and being intimate with his Family and correspondence through the whole term of his service in the army, and that he is not interested in this claim.

Sworn to an subscribed before me this day by the above named affiant and I certify that I am in no wise interested in his claim for which this affidavit is required.

On this 1st day of September, A.D. one thousand eight hundred and eighty-five, personally appeared before me E. B. Hubbard, County Clerk and Clerk of Commissioners Court the same being a court of record within and for the county and State aforesaid, Margaret A. Carley aged Forty years, who, being duly sworn according to law, makes the following declaration in order to obtain the pension provided by Acts of Congress granting pensions to widows: That she is the widow of Lot H. Carley who enlisted under the name Lot H. Carley at Dubuque, Iowa, on the 27th day of August, A.D. 1861, in Company "E" Fifth Regiment of Iowa Cavalry in the war of 1861, who whose death was caused by General debility, as after effects of Chronic Diarrhea and Scurvy contracted during Prison life in  Andersonville Prison, who died on the 15th day of July, A.D. 1885, who bore at the time of his death the rank of 1st Lieutenant, in the service aforesaid; that she was married under the name of Margaret A. Daigh to said Lot H. Carley on the 18th day of February, A.D. 1964 by Rev. William Studdard at Fairplay, Wisconsin, there being no legal barrier to such marriage; that neither she nor her husband had been previously married; that she has to the present date remained his widow; that the following are the names and dates of birth of all his legitimate children yet surviving who were under sixteen years of age at the father's death, to wit:

That she has not abandoned the support of any one of his children, but that they are still under her care or maintenance, that she has not in any manner been engaged in, or aided or abetted, the rebellion in the United States; that A prior application has been filed by Lot H. Carley and a pension granted him numbered 226005, that she hereby appoints J. W. Tallmadge, Washington D.C. her attorney to prosecute her claim; that her residence in No. ______ street, Auburn and that her post office address is North Auburn Nehama County, Nebraska.

Also personally appeared John Daigh, residing at No. ______ in _________ street, in Auburn, and F.C. Richards residing at No. ______, in _______ street, in Auburn, persons whom I certify to be respectable and entitled to credit, and who, being by me duly sworn, say that they were present and saw Margaret A. Carley, the claimant, sign her name (make her mark) to the foregoing declaration; that they have every reason to believe, from the appearance of said claimant and their acquaintance with her, that she is the identical person she represents herself to be; and that they have no interest in the prosecution of this claim.

Sworn to and subscribed before me this 1st day of September, A.D. 1885, and I hereby certify that the contents of the above declaration, &c., were fully made known and explained to the applicant and witnesses before swearing, including the words ________ added; and that I have no interest, direct or indirect, in the prosecution of this claim.

DECLARATION FOR ORIGINAL INVALID PENSION.

On this 21st day of October, A.D. one thousand eight hundred and seventy-nine personally appeared before me, Clerk of the County Court, a court of record within and for the county and State aforesaid, Lot H. Carley, aged 39 years, a resident of the Grove City, county of Christian, State of Illinois, who, being duly sworn according to law, declares that he is the identical Lot H. Carley who was ENROLLED on the 27th day of August, 1861 in company E of the Fifth regiment of Iowa Cavalry commanded by Col. W. W. Lowe, and was honorably DISCHARGED at Clinton, Iowa on the eleventh day of August, 1865; that his personal description is as follows: Age, 39 years; height 6 feet -- inches; complexion, Dark; hair, Dark; eyes, Grey. That while a member of the organization aforesaid, in the service and in the line of his duty at Andersonville Savannah & Millen in the State of Georgia on or about the 30th of July to 20th of November, 1864, he was debilitated by scurvy and diarrhea caused by the insufficiency and improper food & from exposure and bad treatment while confined in this military prison, to wit, Andersonville Savannah and Millen.

That he was treated in hospitals as follows: U.S. Gen Hospital Div. No. 1, Annapolis, Md. Surgeon Vanderkeift in charge. That he has not been employed in the military or naval service otherwise than as stated above. That since leaving the service this applicant has resided in the City of DuBuque, Iowa & Louisville, Ky. & Grove City, Christian County, Illinois, and his occupation has been that of a Plasterer and Farmer. That prior to his entry

into the service above named he was a man of good, sound, physical health, being when enrolled a Cooper. That he is now partially disabled from obtaining his subsistence by manual labor, by reason of his injuries, above described, received in the service of the United States; and he therefore makes this declaration for the purpose of being placed on the invalid pension roll of the United States.

He hereby appoints, with full power of substitution and revocation, T.W. Tallmadge of Washington City, Dist. of Columbia, his true and lawful attorney to prosecute his claim. That he has not received or applied for a pension. That his Post Office Address is, Grove City, county of Christian, State of Illinois.

Also personally appeared H. C. Lindsey, residing at Grove City and J. A. Armstrong, residing at Grove City, persons whom I certify to be respectable and entitled to credit, and who, being by me duly sworn, say they were present and saw Lot H. Carley, the claimant, sign his name (or make his mark) to the foregoing declaration that they have every reason to believe, from the appearance of said claimant and their acquaintance with him, that he is the identical person he represents himself to be; and that they have no interest in the prosecution of this claim.

Sworn to an subscribed before me this 21st day of October A.D. 1879, and I hereby certify that the contents of the above declaration, &c., were fully made known and explained to the applicant and witnesses before swearing, including the words day of state, erased, and the words Dist. added, and that I have no interest, direct or indirect, in the prosecution of this claim.

On this 8th day of November, 1882, before me a Deputy Clerk of Courts within and for the county and State aforesaid, personally appeared Chloe Ann VanVlask, whose age is 55 years, and whose P.O. address is DuBuque, Iowa and Hellen VanVlask Bradley whose age is 38 years, and whose P.O. address is DuBuque, Iowa and being by me first duly sworn, declare that they have been well and personally acquainted for the period of 30 years and 2.5 years, respectively, with Lot H. Carley who claims to have rendered military service as Lieutenant E. 5th Iowa Cav. and during said period have had continuous personal knowledge of his health and physical condition; that during all the period covered by the affidavit said Lot H. Carley was disabled by the effects of chronic diarrhea and scurvy contracted in Rebel Prison by bad food and improper treatment so that he was not able to perform the labor of an ordinary able-bodied man, but that, on the contrary, during the period covered by this affidavit said soldier was confined to his bed at least cannot estimate of the time; and that taking the time right through from month to month and from year to year for the period covered by this affidavit, the said soldier did not and could not do or perform to exceed one-third as much labor as an ordinary able-bodied man; that said Lot H. Carley was not, to affiance' knowledge, afflicted with any other disability save that recited during the period covered by this affidavit.lo

Affiance further declare in reference to said soldier's condition during the period covered between the Eleventh day of August 1865 and the First day of December 1862 to be as follows:

That for the first three months after discharge he was confined to his bed one-half of the time and was not able to perform any labor of any kind for the three months from that time to the fall of 1864 he has been unable to perform manual labor for weeks at a time and has not been at any time in sound health since discharge. That during the period covered by this affidavit said Lot H. Carley was by occupation a cooper & laborer and that he always sustained a good reputation for the truth and veracity, was a man of good moral character, and did not aggravate or prolong his disabilities by intemperance or other bad habits. That these statements are all based on affiant' personal knowledge of the things herein recited, and that affiant' means of having personal knowledge of these things were as follows:

I having a visiting acquaintance with applicant and family and seeing and meeting with him often, as often as once a week and living further than six miles apart and some times in the same house he boarded at our house the first three months after discharge.

Subscribed and sworn before me this day by the above named affiant, whom I certify to be reputable and entitled to credit. I further certify that I am in nowise interested in the claim of the said Lot H. Carley for pension; and the contents of the above affidavit were fully explained and read to affiant before signing.

Respectfully returned to the Commissioner of Pensions. Lot H. Carley (Coral.) Company "E", 5th Regiment Iowa Cavalry Volunteers, was enrolled on the 27th day of August, 1861, at Dubuque for 3 years, and is reported: Re-enlisted as a Veteran Vol. January 1st/64 August, 31/64 (1st Sergt.) Prisoner of War since July 31/64. (Co. was in action that date at Sewnan, Ga.) Same report to Dec. 31/64. Feby. 28/65 2nd Lt. present. He was discharged to date Feby. 27, 1865 by reason of promotion to 2nd Lieut. same Co. and Regt.

Prisoner of War Records show him captured at Rock Mills, Ga. July 31, 64. Paroled at Savannah, Ga. Nov. 20, 64. Admitted to Hospital Div. A. Annapolis, Md. Nov. 26, 64. Furloughed Dec. 10, 64 for 30 days. No later record and no evidence of disability as alleged.

Sir:

I have the honor to acknowledge the receipt of your request on August 8th, 1881, for certain information for use in the consideration of application for pension No. 341,726, and to return it herewith, with the following information from the records of this office:

It appears from the rolls, &c., on file in this office that Lot H. Carley was mustered into the service of the United States as 2nd Lieut. Co. E, 5th Regiment of Iowa Cavr, to date Feby 3, 1865, to serve unexfio herein and mustered out with Co. Augt. 11, 1865.

Before me, A County Clerk in and for the aforesaid County and State duly authorized to administer oaths, personally appeared Lot H. Carley, to me well known to be reputable and entitled to credit, who being sworn according to law, defuses as follows:

That from the hardships he experienced in Rebel Prisons he contracted diarrhea and scurvy, which so greatly debilitated him as to shatter his constitution for life; and that ever since his imprisonment up to the present time he has been continuously disabled with chronic diarrhea and scurvy which affect him in the following manner:

has frequent Billions attacks, diarrhea and when checked, constipated with headache and gen frustration which unfits him for labor at least one half the time is not able to perform manual labor. That he is unable to prove his soundness before enlistment by the evidence of a physician because of one being dead and one being in prison not having been treated by any others. That from the time of his discharge up to 1868 he was treated by Dr. Harney and Dr. McBrien That Dr. McBrien died on or about 1876 and that Dr. Harney is serving out a life sentence in prison at Wanpon, Wis.

That accordingly he files the evidence of neighbors to prove his soundness before enlistment and condition after discharge, as the best witnesses he can offer in place of medical testimony as they are reputable persons and had abundant opportunities of knowing all the facts to which they testify.

State of Iowa, County of DuBuque SS

On this 11th day of April A.D. 1882; personally appeared before me, a Deputy Clerk of Dist. Court in an for the aforesaid county, duly authorized to administer oaths, Thomas Allen aged 41 years, a resident of DuBuque in the County of DuBuque and State of Iowa and Josiah Conzott aged 43 years, a resident of DuBuque in the County of DuBuque, and State of Iowa who being duly sworn according to law, state that they are acquainted with L. H. Carley applicant for invalid Pension, and know the said L. H. Carley to be the identical person of that name who enlisted or volunteered as a Private in Company "E" Fifth Regiment of Iowa Va. Vols., and who was discharged at Nashville, TN. on or about the Eleventh day of August 1865 by reason close of the war.

That the said L. H. Carley, while in the line of his duty, at or near ???? in the State of Georgia did, on or about the 30 day of July 1864 become disabled in the following manner, while on a Cav. raid in the rear of the attack cutting railroad with Gen. McCook he was captured by the rebels and confined in Andersonville Savannah & Millen Rebel Prisons for near four months from July 30 to Nov. 20, 1864 with no shelter night or day & an insufficiency of food and poor quality, did contract scurvy & chronic diarrhea to an extent that he was unfit for duty for near three months after his Parole on Nov. 20, 1864.

That the facts stated are personally known to the affiant then as after by reason of being with the command at the time of his capture. The general conceded fact from letters, reports &c. he being a Corral. & in command of all the Co. that escaped at the time of claimants capture. That the facts stated are personally known to the affiant Josiah Conzott by reason of his being Orderly Sergeant & keeping the Co. records making out muster rolls &c.

The affiant further state that the claimant was sound & health and free from scurvy and chronic diarrhea at the time of his capture and that they did exist after his return to the command after his parole. And deponents further state that they are well acquainted with the claimant, having known him for at least twenty-four years and further, that their knowledge of the facts above stated was derived from said acquaintance, and from having served as members of Company "E" of the Fifth Iowa Regiment of Calvary volunteers from the 27th day of August 1861 to the Eleventh day of Aug 1865. And deponents further state that the claimant was a sound and able-bodied man at and prior to enlistment and capture so far as they knew, and that they are totally disinterested in this claim.

Sworn to and subscribed before me this day by the above named affiance, and I certify that I read said affidavit to said affiance including the words _____ erased, and the words _____ added and acquainted them with its contents before they executed the same, I further certify that I am in nowise interested in said case, nor am I concerned in its prosecution; and that said affiance are personally known to me and that they are credible persons.

Personally came before me, a Clerk of the Circuit Court in and for the aforesaid County and State, J. G. Harvey a citizen of Grove City, County of Christian, State of Illinois, well known to me to be reputable and entitled to credit, and who being duly sworn, declares in relation to the aforesaid case as follows:

That he is a practicing physician, and that he has been acquainted with claimant for about 4 years, and that He did not know soldier prior to enlistment having known him over 4 years. Has been his family physician over 4 years and lives on opposite side of street from him, a new neighbor. He has treated soldier since discharged, first Oct. 11/1874 for Billions fever and ciphialgia, has prescribed for him occasionally since that time to the foresaid for cephalalgra and general debility. His constitution has received a severe shock from some cause prior to his acquaintance with him, from which he suffers. He is not able to perform the labor of a perfectly sound man since he has known him, and is frequently prostrated with severe attacks of ciphalalgia.

He further declares that he has been a practitioner of medicine for 17 years, and that he has no interest, either direct or indirect, in the prosecution of this claim.

Sworn to and subscribed before me this 14th day of April A.D. 1879, and I hereby certify that the affiant is a practicing physician in a good professional standing; that the contents of the above declaration, &c., were fully made known to him before swearing, including the words __________ erased, and the words ________ added; and that I have no interest, direct or indirect, in the prosecution of this claim.

Alleges in declaration filed February 3rd, 1880, that at Andersonville Prison Ga. from July 30 to Nov 21st 1864 from exposure and mivations contracted scurvy and diarrhea. Also at Millen and Savannah Prisons.

ARREARS OF WIDOS'S PENSION.

Allow arrears of pension at the rate of $8.00 per month from July 16, 1885, and two dollars a month additional for each child, as follows:

Was pensioned April 5th 1886 at $8.00 per month from September 11, 1885 and $12.00 per month from March 19, 1886 and two dollars a month additional for each child as above stated.
